There are three versions of our New Membership form. Prior to December 31, 2025 the club is offering promotional pricing for new memberships. Please click on the button applicable to your situation to download a PDF form to provide your new member information.

Current members are encouraged to send these forms to prospective members or provide prospective members with the appropraite web site links. provide to the club the information for individuals or couples you would like to propose for membership.

The completed forms should be mailed to 25 Highland Park Village, Suite 100-555, Dallas Texas, 75205-2789.

If you are mailing in the form, please provide a brief cover letter to identify yourself and relate why you feel that the member candidate might be willing to join the club.  Information regarding whether they have come as a visitor or have connections to other members would be helpful.
